Phentermine should not be taken by people with the following conditions: History of heart problems, such as coronary artery disease, stroke, arrhythmias, heart failure, or uncontrolled high blood pressure Hyperthyroidism Glaucoma History of drug abuse Agitation Pregnancy or breastfeeding Allergic reaction to phentermine or a similar drug Because of the risk of severely high blood pressure, phentermine is also contraindicated in those who take or have taken MAOIs within the last 14 days
Women of reproductive potential should be counseled about these risks and should use effective contraception during treatment

However, this initial decrease comes mainly from fluid loss, appetite/eating changes, and potential GI side effects not necessarily fat loss
